- Polo shirts and scrub tops are limited edition for Maui Health's 5-year anniversary.
- Polo shirts and scrub tops will be delivered to departments.
- Please make sure to include the correct location and department on the order form.

- Each employee may order either one (1) polo shirt OR one (1) scrub top.
